Primary Function

Position will serve as the Supervisor for the Medication Assisted Treatment (MAT) Program for CIHA Behavioral Health. The Medication Assisted Treatment Program is the use of medications with counseling and behavioral therapies to treat substance use disorders and prevent opioid overdose. Supervisor will provide clinical support, administrative and clinical supervision for Medication Assisted Treatment Program staff. Clinical supervisor will oversee the documentation and professional issues of Opioid Treatment Program staff. This position will review and assess client treatment plans, including short- and long-term goals and clinical records to evaluate the necessary level of patient care.  This position will primarily serve adults but may also see families and children. This position will be 70% clinical and 30% administrative. Services include strength based comprehensive clinical assessments of adults and children, implementation of appropriate evidenced based treatments for psychiatric and substance use disorders when indicated, case management services and collaboration with other professionals in an integrated multidisciplinary environment.  Employee is expected to provide individual, family, and group therapy utilizing culturally appropriate and evidence-based modalities of treatment. Employee will provide emergency, crisis intervention, consultation and education services as needed.

Supervisory Duties

• Provide clinical and administrative supervision to Medication Assisted Treatment Program staff.
• Position is expected to be 70% clinical and 30% administrative
• Take direction from the Adult Services Manager and the Behavioral Health (BH) Clinical Director.
• Understands the harm reduction philosophy of treatment and portrays this in leadership.
• Ensure CIHA meet all applicable standards and requirements.
• Ensure CIHA Opioid Treatment Program meets or exceeds productivity and revenue targets.
• Ensure knowledge of all applicable CIHA/Analenisgi Policy and Procedures.
• Provide clinical oversight for all patients served with BH and substance use needs who are enrolled in the Medication Assisted Treatment Program.
• Provide clinical supervision to staff as needed.
• Facilitate weekly multidisciplinary and cross agency staff meetings which may involve CIHA behavioral health and interdepartmental staff and stakeholders.
• Ensure Medication Assisted Treatment Program is taking measures to improve practice by completing performance improvement projects in areas needing improvement.
• Inform leadership of needs of the Opioid Treatment Program as they arise and make recommendations to resolve
• Participate in leadership/supervisory training and activities to enhance competencies in areas as needed or assigned.
• Participate in completing annual performance appraisals of direct reports with Adult Service Manager.
• Ensure quality evidenced based treatment interventions are utilized with clients.
• Collaborate with all BH teams including, but not limited to the BH child/adult services, psychiatry, nursing, behavioral health clinical director/assistant director, peer support and targeted case management.
• Ensure collateral (family/supports) involvement in treatment/safety planning for patient when appropriate.
• Ensure timely, accurate and complete communication to residential treatment staff and interdepartmental teams on transfer of patients to Kanvwotiyi.
• Ensure clinical data is collected and reported as required according to the (BH, ER and Inpatient) department’s specific needs and CIHA policy.
• Monitors and maintains utilization management of any services, as determined.
• Ensure documentation of all treatment and clinically significant events in Electronic Health Record as prescribed by CIHA policies and procedures and quality assurance standards.
• Communicate regularly with the Behavioral Health Director, Behavioral Health Assistant Director, Adult Services Manager, Child Services Manager, CIHA Executive staff, and other hospital departments as needed.
• Provide staff training as directed, according to expertise and scope of practice.
• Participates in the provision of periodic in-service training for non-health care practitioner members of the clinical and administrative staff on subjects germane to clinical and case management, services. This shall include responsibility for developing and implementing psycho educational programs, community relations, information and referral.
• Participate in completing performance appraisals of direct reports with applicable manager/supervisor.
• Participates in division/program meetings to review and identify opportunities to improve the program and treatment provided, and recommend corrective action when problems are identified. This includes assisting with changes in policy and procedure, which would improve the care of service delivery
• Assists in attaining and maintaining accreditation by the Joint Commission of Health Care Organizations.
• Ensures implementation of provisions contained in all applicable privacy/confidentiality policies, procedures, and practices for maintenance and release of patient records and clinical information, including EBCI, CIHA, IHS, NC DHHS, HIPPA, Joint Commission
• Assist with recruitment and hiring process for employees.
• Develop and maintain collaborative relationships with community stakeholders.
• Assist in other areas of BH department when needed.
• This position requires direct service duties with time allocations given to complete supervisory duties.
• Participate in a rotation for on-call crisis services as part of the BH Team
• Other clinical and administrative duties as assigned.

Direct Service Duties

• Provides services to EBCI and other eligible persons on primarily outpatient basis withsome inpatient consultation. Responsible for completion of intake forms, identification of community resources, patient comprehensive clinical assessments, treatment plans, provision and coordination of supportive services, patient advocacy.
• Provides therapy services to patients in exceptionally difficult cases where such is appropriate as determined by treatment needs.
• Assess and treat clients in individual, group, couples, and family modalities exercising professional, clinical judgment and using a wide range of therapeutic skills to assist patients and their families in dealing with chronic and acute MH/SUD diagnoses and trauma.
• Cooperates and collaborates with the medical staff and other health professional/agencies in developing a viable therapeutic team network to facilitate and establish patterns of cooperative efforts and to facilitate patient care, discharge and follow-up.
• Provide counseling to patient and family in matters directly related to patients' limitations, finances, adjustments to medical condition and ongoing treatment.
• Implements and follows the provisions contained in all applicable privacy/confidentiality policies, procedures, and practices for maintenance and release of patient records and clinical information, including EBCI, CIH, IHS, NC, DHHS, HIPAA, Joint Commission, CFR 42, or other licensing bodies.
• Responsible for participating in clinical staffing as an integral component of the Opioid Treatment Program team, facilitating the discussion of patient data, and formulating changes to the treatment plan as clinically indicated.
• Responsible for the development of individual treatment plans including development of time limited, quantified, measurable, and salient goals, and prescribing procedures and interventions which facilitate goal attainment. Treatment plans must be consistent with the patient diagnosis, agency quality assurance guidelines, and level of care policies.
• Shall perform limited administrative duties, which include maintaining statistical records of clinical workload, utilization review, and quality assurance, participating in education programs, and participating in clinical quality assurance functions.
• Assists in attaining and/or maintaining compliance with CIHA, Joint Commission, IHS, Medicaid, SAMHSA, other required governing bodies and funding sources
• Timely record all treatment and clinically significant events for the medical record in the manner prescribed by CIH policies and procedures, quality assurance, quality improvement, and documentation manuals.
• Provides written, telephone, and face to face consultation services to various persons and organizations which affect the patient. These include schools, courts, probation officers, physicians, hospitals, medical care providers, law enforcement officers, social services and community agencies.
• Represents the Medication Assisted Treatment program at regular team meetings for progress review and during emergency meetings dealing with high-risk situations
• Utilize training and skills in community organizations, participate in community planning and implementation of programs to help address such complex issues as improving coordination of services or developing programs for individuals with intellectual and developmental disabilities, and persons with substance use disorders.
• Expected to obtain continuing education both through in-service training and through access and tuition payment for approved training seminars and workshops.
• May provide on call crisis services on a rotating basis with other staff.
• Participate in treatment program cross training, in order to assist with fulfilling other programmatic needs.
• Ensure collateral involvement in treatment/safety planning.
• Ensures essential services coverage for those needed for the OTP.
• Ensure documentation of all treatment and clinically significant events in Electronic Health Record as prescribed by CIHA policies and procedures and quality assurance standards.
• Adhere to all applicable confidentiality policies, procedures, and practices for maintenance and release of patient records.
• Responsible for completing CIHA BH established forms needed, identifying and referring to community resources, CCA and treatment planning provision and coordination of supportive services, patient advocacy.
• Assess and provide treatment for patients in individual and group modalities, exercising professional judgment and using a wide range of therapeutic skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute behavioral health/substance use diagnoses.
• Learn and effectively utilize the electronic health record and other software required for operation in the CIHA system.
• Adhere to ASAM or other appropriate criteria when recommending and referring client to level of care
• Assists with observed urine drug screens as outlined in Analenisgi policies and procedures.
• This position is expected to provide sufficient billable/reimbursable services to achieve the productivity standard established for this position.
• May rotate with staff to provide walk-in access to care services daily, if needed. Triage clients to determine priority, emergent, urgent, routine and work with team to ensure client is served in timely manner.
• Depending upon location of services maintain knowledge of all policies and procedures relevant to respective location; i.e., Cherokee Tribal Justice Center, Snowbird and Marble Clinics, CIHA inpatient, Analenisgi, Primary Care clinic.
• Covers other CIHA covered clinical locations when assigned i.e. Kanvwotiyi.
• Other duties as assigned.

   

  Education/Experience/Qualifications

  Required

• Master’s degree in social work, or counseling required; or doctoral degree in psychology.Degree must be obtained from an accredited university that is accepted by appropriate licensing agencies in NC; and
• Must be fully licensed in NC in specific discipline including Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor(LPC) or Licensed Psychologist (PhD);
• Must be fully licensed as a Licensed Clinical Addictions Specialist (LCAS)
• Must show competence in Behavioral Health and Substance Use Disorders by licensure, training or experience; and
• Must be approved for and maintain medical credentialing, upon hire; and
• Must possess valid drivers' license.

• CCS Preferred
• Specific experience working with Native Americans.
• Experience in working in a medication-assisted treatment program.
