Job Responsibilities & Qualifications:
PRIMARY FUNCTION:   The incumbent performs nursing procedures to patients of all ages independently within the framework of established policy.

Job Description: 
Provides nursing care in accordance with established practical nursing care standards, participates in emergency care, routine and specialty clinics, curative, rehabilitative, and preventive nursing care.  May be required to rotate between nursing departments including outpatient, inpatient, and the emergency room and between shifts, also may be subject to on-call and callback.

Performs ongoing assessment by actively participating in the determination of meeting the nursing care needs based upon collection and interpretation of data relevant to the health status of the client.
Collects data from relevant sources regarding the biological, psychological, social and cultural factors and determines the influences these factors have on the health status of the individual.
Collects data through observations of appearance and behavior, measurements of physical structure and physiological function, and information regarding available resources. 
Interprets data and recognizes existing relationships between data collected and the client’s health status and treatment regimen and determines the client’s need for immediate nursing interventions. 
Plans nursing care activities related to the findings of the nursing assessment.
Participates in making decisions regarding implementation of nursing intervention and medical orders and plan of care through the utilization of assessment data.
Participates in multidisciplinary planning by providing resource data.
Identifies nursing interventions for review by the registered nurse.

Implements nursing care according to the established health care plan and as delegated by the registered nurse.
Performs nursing interventions including, but not limited to:
o Venipuncture and insertion of a peripheral vascular access device.
o Administration and monitoring of I.V. fluids and medications via peripheral lines per hospital policy and State Board of Nursing regulations.
o The administration of medications including topical, enteral, and parenteral routes per manufacturer guidelines and established protocols.  Documents all known drug allergies on the medical record and according to protocol.
o Collects diagnostic data such as: obtaining measurement data, visual acuity and vital signs; collecting stool, urine, and sputum specimens.
o Assist with diagnostic and therapeutic procedures such as Pap smears, spinal taps, proctoscopic examinations, application of casts, minor surgery, etc.
o Performs procedures according to protocol such as EKGs, urinalysis, and random blood sugars.
o Prepares the client for examination and assist the primary care provider in treatment.
o Provides teaching and counseling reinforcing activities for clients as planned and initiated by the registered nurse or primary care provider.

Participates in the development and implementation of quality improvement activities.

Recognizes responses to nursing interventions and modifies immediate nursing interventions based on changes in the client’s status.

Evaluates desired outcomes of nursing care and planning for subsequent care by collecting evaluative data, recognizing the effectiveness of nursing interventions, and proposing modifications to the plan of care for review by the registered nurse or primary care provider.

Communicates significant information to the registered nurse or primary care provider within a time period, which is consistent with the client’s need for care.

Documents all significant information on the appropriate client record, nursing care plan or other required documents. 

Collaborates in implementing the health care of the client with individuals whose services may have a direct or indirect effect upon the client’s health care.  

Participates in planning and implementing nursing or multidisciplinary approaches for the client’s care, seeks and utilizes appropriate resources in the referral process and safeguards confidentiality.

May be necessary to work when Administrative leave is granted if patient care would be compromised.

EDUCATION, LICENSURE, CERTIFICATION, AND EXPERIENCE
Applicant must have graduated from an accredited Practical Nurse program and has obtained licensure (LPN) in the state of North Carolina. 
Current Basic Life Support (BLS) is required for the position. Advanced Cardiac Life Support (ACLS) may be substituted. Can be acquired through the facility following appointment to position.
Proficiency rated at 12 weeks.


JOB KNOWLEDGE
Knowledge of principles, practices, and techniques of nursing in order to provide nursing care to meet the physical, emotional, spiritual, and socio-cultural needs of patients of all ages; specifically, Infancy (Birth-1 year), Toddler and Early Childhood (2-5 years), Latency (6-9 years), Preadolescence (10-12 years), Early Adolescence (13-15 years), Adolescence (16-20), Adult (21-64 years), and Elderly (65-+ years).

Knowledge of the nursing process and its application to clinical practice.

Knowledge of the disease process and illness in order to recognize complications.

Knowledge of medications and dosages in order to recognize adverse side effects.

Knowledge and ability to organize work in order to meet priorities and accomplish work within established time frames.

Knowledge of the occupational functions of multi-disciplinary health care team.

Knowledge of the culture and medical health profile of the patient population.

Knowledge and ability to provide follow up teaching to patient/family on health maintenance and disease prevention.

Knowledge of available health care programs and community resources.

Knowledge and skills in operating specialized medical equipment, such as incubators, suction pumps, oxygen equipment, etc.

Knowledge of problem oriented medical record methods.

Applicant must have a valid North Carolina driver’s license.

COMPLEXITIY OF DUTIES
Work requires the performance of a full range of general nursing care, consisting of routine to complex nursing assignments. This includes administration of medications, therapeutic measures, operation of medical equipment, evaluation of care to ensure that the physical and psychosocial needs of the patient are met and provides follow up teaching to patient and families.  Makes decisions as to proper procedures based on the type of patient and needs of that patient.

SUPERVISION RECEIVED
Incumbent works under the supervision of the nurse supervisor, charge nurse, or designee.  Nursing procedures are carried out independently within the framework of established policy.  Work is reviewed by the supervisor for adherence to professional standards and for overall effectiveness in meeting the objectives of the department. 
Guidelines consist of professional standards of practice as determined by the nurse practice act, nursing and medical policy and procedures, standing orders, and hospital and agency policies.  Must, also, comply with FPM, DHHS, PHS, and IHS regulations.  These guidelines are not always specifically applicable to the individual patient or situation and independent judgment is required in selecting the most appropriate guideline, and applying the intent of the guideline to the specific situation at hand.  The LPN receives assistance from the supervising nurse or his/her designee with significant deviation from available guidelines or the lack of guidelines.

RESPONSIBILITY FOR ACCURACY
The incumbent’s performance affects the health, recovery, and rehabilitation of patients,
and the quality of care provided.  Evaluations and observations are used to modify and
develop clinically appropriate treatment plans.  The incumbent has a positive effect upon
the recovery of the patient and is responsible for following policies and procedures,
which serve as hospital guidelines.

CONTACTS WITH OTHERS
Contacts are with patients, families, hospital personnel, and with a variety of Indian Health Service, other tribal and non-tribal community agencies.
Contacts with patients, families, and hospital personnel are to exchange, provide, and obtain information concerning the patient’s physical and psychosocial health care problems, and needs.  The LPN uses teaching and counseling methods to influence and motivate patient and family behavior.  Contacts with other health care or related disciplines within and outside the hospital are for the purpose of collaboration and consultation.

CONFIDENTIAL DATA
The Privacy act of 1974 mandates that the incumbent shall maintain complete confidentiality of all administrative, medical, and personnel records and all other pertinent information that comes to his/her attention or knowledge.  The Privacy Act carries both civil and criminal penalties for unlawful disclosure of records.  Violations of such confidentiality shall be cause for adverse action.

MENTAL/ VISUAL/ PHYSICAL
Work in the various services within the nursing department requires considerable walking, standing, bending, pushing, and lifting in helping patients to and from beds, wheelchairs, and stretchers.  These same activities are required in moving equipment and medical supplies.

ENVIRONMENT
The work environment involves moderate risks of exposure to infectious disease, radiation, electrical hazards, irritant chemicals and explosive gases.
