Primary Function    

Provides personal care and performs routine tasks for facility residents of the Tsali Care Center as assigned by supervisor, and all related duties of Certified Nursing Assistants as certified by the State of North Carolina.


Job Description

  • Implements the ordered plan of care for residents in the assigned caseload and schedule.
  • Is required to work on weekends and holidays on a rotating basis.
  • Takes vital signs as specified in CNA Plan of Care, understands normal parameters and reports results outside of normal parameters to residents' primary RN or PCS supervisor immediately if indicated and otherwise within the working day.
  • Organizes daily schedule to make good use of time and care of residents.
  • Demonstrates safe practices by using proper body mechanics, following policies for sharp/dangerous material handling and disposal, reporting and/or acting to correct unsafe or costly conditions.
  • Cleans and restocks CNA supplies daily and as needed to maintain current and complete nursing assistant supplies.
  • Maintains current CPR certification, TB test, and CNA certification without lapse and provides copies to supervisor.
  • Attends all mandatory agency in-service/education programs as scheduled during the evaluation period.
  • Attends at least 12 hours/year of work-related educational programs.
  • Participates in QA and Performance Improvements projects as assigned.
  • Attends monthly staff meetings and scheduled case conferences involving assigned residents, unless excused by PCS Supervisor. Documents the reading of minutes when unable to attend staff meetings.
  • Performs other duties assigned by supervisor to promote efficient operation of the facility.

    Education /Experience         

  • A high school diploma or GED is preferred.
  • Must already be a Certified Nursing Assistant or have previous medical experience with the willingness to obtain CNA certification within 4 months after being hired to qualify for this position. Will be considered a Patient Care Assistant (PCA) until the Nurse Aide Certification is obtained and follow the job description of a PCA until that point.Pay will also be at a PCA rate.
  • Three to six months would be necessary to become proficient in most phases of this position.
  • Experience providing care to ill or elderly would be helpful.
  • Must possess sufficient and legible written and verbal skills in order to communicate with residents and health care providers.
  • Must maintain specified certifications.

Job Description
The Personal Care Assistants (PCA- Non Certified) will partner with Certified Nurse Assistants (CNA’s) to provide routine direct assistant care to assigned patients following successful completion of at least 16 hours of training in the following areas: communication and interpersonal skills; infection control; safety/emergency procedures; including relief of choking; promoting residents’ independence; and respecting residents’ rights. A minimum of 16 hours of practical (combined laboratory and clinical) training must be accomplished prior to any resident care.


Career Ladder/On-The-Job Training

The Personal Care Assistant will work under the direct supervision of the nursing staff and in collaboration with the Certified Nursing Assistants providing Activities of Daily Living (ADL) services to residents. During this on-the-job training period, Cherokee Indian Hospital and the Tsali Care Center will work with the Personal Care Assistant to further improve and expand his/her resident care skills through the Nurse Aide I Program and preparing the person to become a Certified Nursing Assistant.

Minimum Eligibility Requirements

You must have a desire to want to be a certified nurse aide and have a heart to help others. Qualities that are important for this job include kindness, reliability, and a willingness to work with elderly adults Must be able to relate to and work with the ill, disabled, emotionally upset and, at times, hostile people within the long term care facility and at times within the community with family members.

  • Assist residents with activities of daily living once the PCA has been deemed competent in the lab and clinical environment to provide the service, including:

    Bathing with the assistance of another for showers, dressing, grooming (oral care), toileting for residents that are ambulatory or with the assistance of another person, changing of bed linens, positioning, transfer with the assistance of another person, mobility, and incontinence care.

  • Focus on service to residents during dining and promote meal time as an enjoyable activity of each resident’s day.
  • Provide assistance to individual residents as needed or directed, who require help with eating once competency has been deemed in the lab and clinical environment and only with residents assigned by the nurse, carrying plates, trays or beverages, cutting up meats or other foods, opening items and placing within patient’s reach.
  • Keep proper records of care as required by the facility. Complete all forms and documentation as required.
  • Immediately report any observations of concern/changes in resident’s condition to the Supervisor in Charge.
  • Vital sign monitoring will be performed as needed or directed by regulations, policy or primary care provider’s order.
  • Be able to apply Tsali Care Center’s accident, disaster, fire safety, elopement and emergency procedures for protection of the residents.
  • Be familiar with State Regulatory Policies and Procedures.
  • Be capable of performing CPR upon successful completion of CPR certification including Heimlich maneuver.
  • Gain knowledge of the residents’ background, preferences, habits and interests and protect the confidential information about each resident.


Safety and Sanitation

  • Monitor all exit doors, the call and alarm systems to assure prompt response.
  • Understand fire watch and need to check exit doors during and after as directed to assure full closure and lock.
  • Maintain the confidentiality at all times of all locked door code systems to assure safety of the residents.
  • Understand and follow safety and infection control policies and procedures and readily report any breach of the safety and infection control procedures to the Supervisor In Charge.
  • Respond to emergency call signals promptly.
  • Report incidents to the Supervisor in Charge promptly and complete appropriate paperwork as required in a professional manner.
  • Readily and directly report any verbal or physical abuse of a resident, inappropriate use of resident or facility property or lack of or inappropriate care of a resident to the Supervisor in Charge, Director of Nursing or the Nursing Home Administrator if necessary.


Laundry/Cleaning Functions

  • Assist with patient’s laundry needs in accordance and as permitted by State regulations and with CIHA procedures.
  • Help to properly maintain all equipment and ensure work areas are clean and safe.
  • Promptly notify the Supervisor in Charge of items that need repair or present a safety risk to residents, employees or visitors.
  • Assist in keeping resident rooms as tidy as possible, picking up items in the dining areas and public areas to keep things clean
  • Notify housekeeping of spills, soiled areas and assist with clean up as per State Regulations as needed. Assure a marker is placed at spill areas until the area is cleaned.


  • Successfully complete Orientation Skills Check Off for the Personal Care Assistant
  • Attend all required meetings and in-service trainings.



  • Participate in, promote and encourage resident’s involvement in activities.
  • Follow and communicate CIHA and Tsali Care Center’s values, goals, policies and procedures.
  • This job description provides a framework for the job; other duties may be assigned as necessary within the Personal Care Assistant’s Scope of Care.

Working Conditions (hours and environment)

  • Is involved with resident, personnel, and visitors under all conditions and circumstances.
  • Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
  • Is subject to injury from falls, burns from equipment, odors, etc., through the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.

Physical/Sensory Requirement

  • Must be able to lift, carry, push, pull, or otherwise move objects that weigh 50 pounds or less.
